To strengthen the Health, Outreach, Prevention, Education (H.O.P.E.+) initiative model as a sustainable, interdisciplinary, focused, cross-sectional health access to primary care and mental health services for vulnerable, and marginalized populations in hard-to-reach communities in El Paso County, Texas.
